your name and phone number(s) so that I may reach you by phone if I cannot reach you by e-mail;
your child's current grade level and the school he/she attends;
Which Judo program (e.g., Port Washington, Great Neck, etc.); and
any additional information you deem relevant or helpful (e.g., any physical or mental issues any teacher should be aware of).
Students: in addition to providing the information above, please include your parent's e-mail address so that I may copy him/her on my reply message.
